Please print out and turn in to Mrs. Hein. Copies are also located on the wooden cabinet between Mrs. Hein and Mr. Cole's offices.

Your physician will have copies.

If you have a physical on file with the school from a sport, you do not need to get a new one UNLESS it will be older than one year during the marching season. Please let us know if you have one on file with the school.
